Populate reading_items.body_excerpt from existing content column

Iterates reading_items and applies htmlToText to derive body_excerpt from content. Default path only touches rows where body_excerpt IS NULL. Pass force:true to re-derive every row (necessary after bumping the htmlToText cap); pair with offset for chunked pagination.
